New Hampshire Heart Disease and Stroke Prevention 2015-2020 Action Plan

In June of 2014, the NH Division of Public Health Services convened a statewide meeting of representatives from public and private organizations serving various roles in community health-related coalitions. The focus of the meeting was to develop an action plan to address the goals identified in the State Health Improvement Plan for the priority area of heart disease and stroke. Additional input was sought from stakeholders throughout the development process.
